Use this form to formally request a patient’s medical notes. This page is only to be used by veterinary clinics or pet insurance providers.
You will be required to declare that you have consent from the client to whom the medical notes pertain.
Medical notes will not be forwarded to private addresses.
Due to the high volume of applications, it may take up to 24 – 36 hours before you receive the medical notes. If your request is urgent, please call the clinic to speak with a veterinarian on duty.
